NSS Guidelines on Submission of NSP Monthly Evaluation Forms

The National Service Secretariat (NSS) has issued guidelines and instructions to personnel regarding the submission of their monthly evaluation forms.

With the aim of streamlining the process, these guidelines are designed to prevent any potential issues that may arise from incorrect completion or submission of the forms.

In order to successfully complete the monthly evaluation form, it is crucial to adhere to the following guidelines:

Required Features:

All evaluation forms must have a QR code at the top right, the NSS logo at the top leftt, and a bar code at the bottom right of the document.

• Bar code at the bottom right

It is important to note that printing the form from a mobile phone is strictly prohibited as all the features might not appear.

Forms should be printed from either a desktop or laptop computer, or individuals can visit a designated CAFE to print the form.

Completion Requirements:

it is of utmost importance to complete the form using a blue pen.

The use of a black pen is strictly prohibited.

If any part of the form, either by the individual or their supervisor, is completed with a black pen, the form will not be accepted.

Supervisor’s names and ranks must be written in full: individuals are required to inform their supervisors to write their names and ranks in full, without using any abbreviations.

Ensure supervisor’s signature is included: any form that does not bear the signature of the supervisor will be rejected.

 It is crucial to obtain the signature of the supervisor to validate the information provided on the form.

Institutional Stamp:

supervisors are instructed not to use the stamp of any other institution on the form, except for the institution where the individual was posted.

 Failure to comply with this requirement will result in the rejection of the form.

Book Appointment:

individuals are advised to log into their dashboard at https://portal.nss.gov.gh/sign-in  and book an appointment after their supervisor has endorsed the form.

That is, when personnel are instructed to submit the forms, they must first book an appointment on the designated day of submission.

 This will allow the district/municipal office to capture their facial features using the biometric/facial recognition device in their dashboard.

The National Service Secretariat has encouraged all national service personnel to comply with these guidelines to avoid any complications during the submission process.
